You do not require a fit note if you are fit or off sick for 7 calendar days or less. You can self-certify for the first 7 days, your employee should accept verbal or written verification or have their own form to do this if you need to claim Statutory Sick Pay (SSP) please click below to access Gov.UK for details and access to the Employee’s Statement of Sickness form (SC2)
